Discuss your eligible uninsured form requirements directly with Dr. Janine Karpakis before choosing a document-review service. Monday–Thursday, 4:30–8:30 p.m. Atlantic Time, subject to availability.
Book a guidance appointmentThe fee and service details are shown in JaneApp before booking. This guidance fee is separate from the form-review prices and assessment estimates below. No insured medical assessment, diagnosis, treatment, prescription or referral is included. Form completion and detailed records review are separate unless expressly included.
Expedited Processing
Most complete requests receive physician review within 5 business hours when complete materials arrive during business hours, Monday–Friday. If approved with nothing further required, the completed form is usually provided within 24 business hours.

What the form-review fee includes
The Full Doctor Review Fee includes the physician’s professional review and, where clinically appropriate, completion of the requested documentation. *Consultation estimates are planning guides
These estimates relate to any additional assessment for a form; they are not the fee for a Medical Forms Guidance Consultation. An assessment is not automatically required. If additional virtual or in-person assessment is necessary, the physician will explain why and confirm the fee before proceeding.
Completion is never guaranteed
A physician may require more records, recommend assessment, or determine that the request cannot be medically supported. Physicians may only sign documentation they can support clinically.
Complexity can change the final fee
Prices are estimates and may vary according to the documentation burden and time required. Any added professional fee is explained before additional work proceeds.
